Application for Commercial Charge Account
All new customers need to complete this form, or sign and attach similar data, in order to be set up in our system.
Certificate of Tax Exempt Sale
Customers that have been given a tax exempt status by the state must submit this form to our Accounting department.
Terms and Conditions of Sale
All customers must have a signed copy of our Standard Terms and Conditions of Sale on file. This outlines a variety of order requirements including but not limited to, Payment Terms, Changes and Cancellations, Delays, Additional Fees and Transfer of Ownership.
Credit Card Payment Form
This form can be completed if paying your invoice by credit card and sent to our confidential Accounting department fax number for processing. A receipt will be emailed to you once this has been processed.
Seating Service Worksheet
This form can be used if issues arise with your seating requiring a service call. Simply complete the worksheet and email to your NBS team member for a resolution.
