HMO members typically choose a primary care physician (PCP) who coordinates their care and refers them to specialists. [ ](https://www.yournbs.com/wp-content/uploads/NBS_2026-BCBSM-PPO-HSA-3400.pdf) ### [Simply Blue PPO HSA (BCBSM)](https://www.yournbs.com/wp-content/uploads/NBS_2026-BCBSM-PPO-HSA-3400.pdf) This plan has a high deductible and requires the patient to pay the entire deductible before any costs are covered by insurance; except for ‘preventive care services’ which are covered 100%. This means that doctor’s visits, prescriptions, tests, etc. are paid in full by the patient as opposed to just paying the co-pay. The advantages include being able to save pre-tax dollars in an account that can be used for qualified medical expenses now and in future years, including after retirement. NBS contributes toward the funding of the HSA account based on the level of coverage elected. NBS has established the HRA to mitigate the financial risks associated with the higher deductible and out-of-pocket maximums.
